I never had the pleasure. I missed him at San Diego Comic Con some years ago, IIRC, because I had strep throat and had to spend a day in bed.
Very sad news, I had the pleasure of meeting him in a convention once. He was one of the nicest, most humble human beings I have ever encountered, a total class act. May God rest his soul and my sincere condolences to his family.
I was sad to see this news earlier in the day. Jeremy had been suffering from Parkinson's disease for quite a while, which turns out to be the reason he retired from conventions as well as private signing a couple years ago. The disease is apparently what led to his death.

Neither actor was "removed" from the films. Jason Wingreen originally voiced Fett & Sebastian Shaw was the "face" of Vader both in the helmet removal and original ghost form.
